Photograph/Video Consent and Release:
I consent to the use of my child's/children/family images, without compensation, whether photograph or video, being used by Parkland Buddy Sports, Inc. in the promotion of its organization and programs. All such images shall be the exclusive property of Parkland Buddy Sports, Inc.
Communication Consent:
I consent to the use of my or my child's email address and/or my phone number (voice and/or Text) to be used by Parkland Buddy Sports, Inc. to communicate with you or your child our sports and event activity information including partner offerings.
City of Parkland Anti Bullying Policy:
The City of Parkland is committed to ensuring that sports organizations that operate within the City provide a caring, friendly and safe environment for their participants. As a result, bullying of any kind is unacceptable and will not be tolerated. Bullying means systematically and chronically inflicting physical pain and/or psychological distress on one or more participants. It is further defined to mean any unwanted purposeful written, verbal, nonverbal or physical behavior including but not limited to any threatening, insulting and/or dehumanizing gesture, by an adult or minor that has the potential to create an intimidating, hostile and/or offensive environment or cause long term damage; cause discomfort and/or humiliation; and/or unreasonably interfere with participation in the sports organization; is carried out repeatedly and is often characterized by an imbalance of power. Bullying may involve, but is not limited to: unwanted teasing, threats, intimidation, stalking, cyber stalking, cyber bullying, physical violence, theft, sexual, religious and/or racial harassment, public humiliation, destruction of property, social exclusion, incitement, coercion, rumor or spreading of falsehoods.
Concussion / Head Injury Policy, Disclosure and Acknowledgement:
A concussion is a brain injury and all brain injuries are serious. They are caused by a bump, blow or jolt to the head or a blow to another part of the body with the force transmitted to the head. They can range from mild to severe, but any concussions are potentially serious and may result in complications including prolonged brain damage and death if not recognized and managed properly. Most sports concussions occur without loss of consciousness. Signs and symptoms of concussion may be displayed right after the injury or may take hours or days to fully appear. If your child reports or displays any symptoms of concussion, or if you notice the symptoms of concussion yourself, you should seek medical attention immediately. If a player, sustains or is suspected of having sustained a concussion, the player will not be permitted to return to the sports activity registered for until a health care professional has examined the player and the player's parent or legal guardian provides a written authorization allowing the player to return to the sports activity.
Waiver and Release:
TO ALL TO WHOM THESE PRESENTS SHALL COME OR MAY CONCERN, KNOW THAT I, as parent and/or legal guardian ("Releasor"), together with their personal representatives, heirs, next of kin and spouses, in consideration of being permitted to participate and other valuable consideration, the receipt of which is hereby acknowledged, releases and discharges Parkland Buddy Sports, Inc. and the City of Parkland ("Releasees"), from all claims, actions, causes of action, suits, debts, dues, sums of money, accounts, reckonings, bonds, bills, specialties, covenants, contracts, controversies, agreements, promises, variances, trespasses, damages, judgments, extents, executions, and demands whatsoever, in law or equity, which against the Releasees, their successors and assigns ever had, now have or hereafter can, shall or may have for, upon, or by reason of any matter, cause or thing whatsoever whether or not caused by the negligence of the Releasees. Whenever the text hereof requires, the use of the singular, it shall also be deemed to mean the plural. This Release may not be changed orally. The Releasor understands and hereby acknowledges that participation in programs provided by Parkland Buddy Sports, Inc. involves strenuous exercise that is designed to strengthen the cardio-vascular system, as well as other muscle groups, but stresses those systems during participation. Releasor acknowledges that the Releasees, and each of them, have recommended that Releasor consult with a physician or other qualified health care provider before participating in programs provided by Parkland Buddy Sports, Inc. Releasor expressly assumes the risk of personal injury or death as a result of participating in programs provided by Parkland Buddy Sports, Inc. Releasor agrees that this Release and Waiver is intended to be as broad and inclusive as permitted by the laws of the State of Florida and that if any portion of this Release and Waiver is held invalid, by a court of competent jurisdiction, it is agreed that the balance shall, notwithstanding, continue in full force and effect. Releasor, being of lawful age, and under no disability, in consideration of being permitted to have their child or children participate in programs provided by Parkland Buddy Sports, Inc. does for herself or himself, as the case may be, together with their heirs, executors, administrators, and assigns, hereby unconditionally release and forever discharge the Releasees from any and every claim, demand, action or right of action, of whatsoever kind or nature, either in law or in equity arising from or by reason of any bodily injury or personal injuries known or unknown, death and/or property damage resulting or to result from any accident which may occur as a result of the participation by Releasor's child or children in programs provided by Parkland Buddy Sports, Inc., whether by negligence or not. Releasor further states that Releasor has carefully read this Release and Waiver and knows and understands the legal effects of this Release and Waiver and signs this Release and Waiver freely and voluntarily. Releasor further releases the Releasees from any claim whatsoever on account of first aid, treatment or services rendered to Releasor's child and/or children during participation in programs provided by Parkland Buddy Sports, Inc.
